It is vital that the most current information regarding your childs health be available to those caring for him or her. Please provide the most recent REPORT OF PHYSICAL EXAM, IMMUNIZATION RECORDS, and any changes in MEDICATIONS or HEALTH HISTORY to the school nurse's office. I can best care for your student, especially in time of emergency, with up to date health information.

Please call and let us know if you will be keeping your child at home. You may phone the ATTENDENCE LINE at 314-415-6676 to report your child's absence, 24 hours a day. For illness tracking, please indicate the reason for illness, such as fever, cough, sore throat or if the child is being treated for an illness such as strep throat, influenza or other communicable disease. If you would rather leave this information with myself directly, please phone my office or e-mail me. Both are regularly checked upon arrival to school and prior to gathering classroom attendence.
