STUDENT HEALTH AND IMMUNIZATION
Letters have been mailed to parents of students who require updated immunizations for attending school beginning the school year 2011-2012. Most students require a tetanus/diphtheria booster shot sometime during high school. Also all students in high school are required to complete a 3-dose Hepatitis B immunization. Missouri State Law does not permit any student to attend school if the required immunizations are not up to date. Students who do not have appropriate updated immunizations on file with the school nurse will not be permitted to attend school as of August 2011.
To all parents who receive the immunization letter, please contact your child’s physician’s office for updating the immunizations. Your child may have already had the immunization but a copy was not provided to the school nurse’s office. Please note that the parents need to communicate directly with the physician’s office as the nurse cannot due to Federal Privacy Laws. Updated records can be faxed directly to the nurse’s office at 314-415-7673.
The Parkway School District recommends a physical exam on entry to the 9th grade. Many changes may have occurred in your child’s health status in the intervening years, or you may not have sent a completed physical exam form to the school. It is important that current information be in your child’s health record. This allows the school nurse to more effectively meet your child’s health needs at school. Accurate information is then also immediately available to Emergency Medical Personnel should an emergency arise.
Included in this bulletin is a copy of the HEALTH INFORMATION UPDATE form, PARENT REQUEST FOR ADMINISTRATION OF MEDICATION AT SCHOOL form, and the PERMISSION FOR STUDENT TO SELF ADMINISTER MEDICATION BY METER DOSED INHALER form to be completed by the parent. Please complete these forms if your child has any health condition that may have an impact on his/her participation in school. This includes any medication taken at home or at school and any illnesses such as asthma, allergies, or any other active illnesses or disabilities. Please also remember that any over the counter medications can be carried by the student in their original containers with a note from a guardian stating that the student has permission to carry that medication.
Also included is a REPORTS OF PHYSICAL EXAMINATION form. All students who are participating in high school sports are required to have a completed physical examination form. This exam should be dated after
February 1, 2011 for the school year 2011-2012. The completed physical form for those participating in sports should be sent to the Athletic Office at North High School.
