Asthma is a chronic health condition that can be safely controlled through the use of medicatons. It is very helpful for the school nurse to have current medical records and Asthma Action Plans for students with asthma. Click here for a copy of the Asthma and Allergy Foundation of America's sample plan that you can print off and take to your child's doctor for completion.

Please bring the completed Asthma Action Plan back to the school nurse and complete the Parent Permission to Administer Medication at School form located on my home page as well.

Both of these forms need to be updated annually.
