Physical Examinations
If your child has a physical examination, please retain several copies of his/her original completed form before submitting it to the school. This will allow you to provide a copy of the documentation for any requests you might receive from activities, clubs or organizations during the year.
Immunization Non-compliance Letters
Non-compliance letters for immunizations will be mailed to all students who need a booster before the 2012-2013 school years. I urge you to take care of this matter early in the summer and send it to the nurse at school as soon as possible so that your student will not be delayed in starting the school year. Your promptness is greatly appreciated.
Senior Immunization Record
Students and/or parents should obtain a copy of the student’s immunization record from Infinite Campus before graduation on May 17, 2012. This is an important document that should be kept in a safe place as many schools and employers require this record; always keep the original document and provide copies to those who may request it.
Asthma
As Spring approaches please don’t forget to supply an inhaler for your student to carry with them as well as an extra inhaler for the nurse’s office.
